This 39-year-old patient presented to Tampa plastic surgeon Dr. Traci Temmen for consultation regarding breast implant removal with mastopexy (breast lift) and fat transfer to the breasts for natural breast enhancement. She underwent breast augmentation in 2010 in Venezuela with approximately 430cc implants, likely silicone, placed via a periareolar incision. She is now ready for implant removal and desires a more natural breast appearance without implants.
She reports intermittent discomfort along the left lateral chest wall but understands that breast implant removal does not guarantee resolution of this symptom. She is accepting of the expected change in breast shape following explant, including smaller, wider, and flatter breast contours, and prioritizes a natural aesthetic outcome. She is not interested in replacement implants and prefers a modest increase in breast volume using fat transfer. She understands that fat transfer typically provides approximately a ½ to 1 cup size increase in breast volume.
Dr. Temmen recommended breast implant removal combined with a superior medial pedicle breast lift (mastopexy) with Wise pattern skin excision to reshape and elevate the breast after explantation. In addition, liposuction of the abdomen, flanks, iliac rolls, and lower back will be performed for autologous fat harvest with transfer to the breasts for natural breast augmentation. She understands that fat transfer will provide subtle volume enhancement and improved upper pole contour, but not implant-level fullness.
Risks and benefits of capsulectomy were discussed in detail, and the patient elected not to proceed with capsule removal. The surgical plan includes implant removal, breast lift, and fat transfer to the breasts as a staged, comprehensive breast rejuvenation procedure after implants.
