In recent years, I have seen a shift in patient perspectives regarding Plastic Surgery. Beauty standards are constantly evolving, and patients adopt this perspective especially when it comes to the eyes. Many patients today seek out refined, natural, and subtle results that will only enhance their natural features, instead of fully transformative procedures. Patients want to look like an elevated version of themselves, not someone completely different. One of my favorite procedures that does just that is upper blepharoplasty. Known as upper eyelid lift or eyelid surgery, the upper blepharoplasty has skyrocketed in popularity, especially in the Tampa area.

The procedure is very minimal, but it can make an extremely meaningful difference in one’s appearance without being blatantly obvious. As a female board-certified plastic surgeon, I find myself having to remind patients that the most successful results are often achieved through the most conservative procedures. The ones that restore harmony to the face, enhancing natural beauty. The upper blepharoplasty is a wonderful example, a simple and elegant procedure that delivers refined results. This procedure reflects the surgical approach I take in a way, focusing on longevity, balance, and natural beauty.

In this blog, I will discuss why perspectives on upper blepharoplasty are changing, what the procedure addresses, common misconceptions, and how patients can determine whether an upper eyelid lift is right for them.

Why Are Perspectives on Upper Blepharoplasty Changing?

Much like trends in body contouring and breast surgery, perspectives on facial procedures have shifted toward subtlety and intention. Many patients today lead active, social, and professional lives and want results that feel lifestyle-friendly and low maintenance. They want to look like the most well rested and refreshed version of themselves. Not the most altered and dramatic version.

The eyes are often one of the first areas of the face to show signs of aging. Because the upper eyelid skin is thin and extremely delicate, it tends to lose skin elasticity faster than other parts of the body. Patients frequently tell me that while they feel energized and healthy, their eyes make them appear tired, stressed, or older than they feel. This disconnect is often what prompts someone to explore upper blepharoplasty.

The biggest change I have seen in patients is wanting the most subtle and refined results, not dramatic ones. These include a more open, rested look, improved definition around the eyelids, and most importantly- results that will age naturally with them. Many patients do not want to deal with the upkeep of additional surgeries over the years, and seek me out for my timeless, elegant style. Additionally, patients want their results to be subtle in a way where friends will say, “You look well-rested!” but unsuspecting of surgery. This “did they or didn’t they?” outcome aligns perfectly with the modern approach to upper blepharoplasty.

What Is Upper Blepharoplasty?

Upper blepharoplasty is a surgical procedure designed to improve the appearance of the upper eyelids by removing a small amount of skin in the upper eyelid crease, and even fat from the inner corner of the eye. Over time, the skin of the upper eyelids can stretch and fold, creating a hooded or heavy appearance that obscures the natural eyelid crease.

For some patients, excess upper eyelid skin can even interfere with vision by resting on the lashes. In these cases, upper blepharoplasty offers both aesthetic and functional benefits. The goal of the procedure is not to change the shape of the eyes, but rather to restore definition, openness, and youthfulness. When performed precisely, upper blepharoplasty enhances the eyes while maintaining facial harmony and their natural shape.

What Upper Blepharoplasty Treats:

Many patients are surprised by how much improvement can be achieved through such a minimal and focused procedure. Because the eyes play such an essential role in facial expression, even subtle changes can have a noticeable impact on overall appearance. Upper blepharoplasty is particularly effective for patients experiencing drooping or hooded upper eyelids, excess skin that conceals the eyelid crease/obstructs vision, a tired or fatigued appearance, or loss of eyelid definition.

The Anatomy Behind Upper Eyelid Aging:

Eyelid aging is not caused by one particular factor. It is simply the result of changes overtime in skin elasticity, muscle support, and underlying structures. As skin elasticity decreases, the upper eyelid skin may begin to fold over the crease. At the same time, a heavier appearance emerges as the muscles responsible for lifting the eyelid weaken. These changes happen at different rates for different people; each body is different. Sun exposure, genetics, and lifestyle factors all play a role, so some patients may show signs of aging earlier/later than others of their age.

Understanding these underlying changes is essential when planning upper blepharoplasty. The procedure is not simply about removing skin, but restoring balance while keeping the natural structure of the eyelid.

Upper Blepharoplasty vs. Brow Lift:

One question I hear during consultations is whether drooping of the upper eyelids should be addressed with an upper blepharoplasty or a brow lift. While these procedures can complement each other, they treat different concerns.

Upper blepharoplasty focuses on excess drooping skin on the eyelid. A brow lift raises the eyebrows, which can in turn raise the upper eyelids and decrease heaviness caused by brow descent. During a consultation I evaluate eyelid anatomy, eyebrow positioning, and overall facial balance to determine the main cause of upper eyelid sagging. Some patients benefit from upper blepharoplasty alone, while others achieve the most natural results through a combined approach. This is different from patient to patient, taking into consideration their anatomy, goals, and realistic expectations.

What Upper Blepharoplasty Cannot Fix:

While upper blepharoplasty is a highly effective procedure to reduce signs of aging, it is important to understand its limitations. This procedure does not treat fine lines or crow’s feet, deep forehead wrinkles, under-eye concerns, or textured skin. Attempting to solve all signs of aging around the eyes with upper blepharoplasty alone can lead to unnatural results, leaving a patient feeling self-conscious and worse. This is why setting realistic expectations is a crucial part of the consultation process. When patients understand what a procedure can and cannot accomplish, they are much more satisfied with their results.

Who Is a Good Candidate for Upper Blepharoplasty?

Ideal candidates for upper blepharoplasty are individuals in good overall health who are bothered by changes in their upper eyelids. While many patients pursue eyelid surgery later in life, genetics can cause eyelid aging to appear earlier, making some younger patients’ appropriate candidates as well. As I mentioned previously, each patient ages at a different rate so there is no “suggested age” for this procedure. During a consultation, I take the time to understand each patient’s goals, lifestyle, and concerns. Upper blepharoplasty is not a one-size-fits-all procedure, and detailed personalized planning is essential for achieving results that both the patient and I are happy with.

The Procedure and Recovery:

Upper blepharoplasty is typically performed in our Tampa office under local anesthesia, but can also be done at our surgical center under general. Incisions are placed within the natural upper eyelid crease, allowing scars to heal discreetly. The scar is virtually undetectable a few months after, by both the patient and surgeon. Most patients experience mild swelling and bruising following the procedure, which quickly improves within weeks.

Patients are often surprised by how manageable the recovery process is. Many can return to daily activities and work a few days after their surgery, though strenuous exercise should be avoided. Any residual bruising can be covered with eye makeup. As swelling subsides, results continue to refine, revealing a more open and refreshed appearance.
